Incoming and Outgoing CDAs, DUAs and MTAs submitted via ServiceNow as of March 1, 2019
Dear Colleagues –
As you’re aware, new CDAs, DUAs and MTAs will be submitted via ServiceNow.
Prior to our ServiceNow forms going live on March 1st, ura.uchicago.edu used an outdated link directing users to an old version of the contract review forms, which is no longer active. The site has since been updated, but we wanted to ensure everyone has a link to the current ServiceNow submission forms. The current link is included below…
Please check to ensure this link above is bookmarked on your browsers for CDA/MTA/DUA submissions.
To confirm successful submission of your contract request, ServiceNow will send an immediate email from email@example.com to your @uchicago.edu address indicating that your request was “submitted.” You should also be able to track the progress of your request at https://uchicago.service-now.com/sso by clicking “My Requests” in the top-right corner of the page. If you do not receive an email with a reference number within 30 minutes of submitting your service, check your “My Requests” tab to confirm your request has been submitted.
